I-9
The I-9 form verifies an employee's identity and establishes that the employee is eligible to accept employment in the United States. This form must be completed on or before the first day of employment. Click here for a list of acceptable documents to present to Human Resources.
W-4
The W-4 form is used to determine the correct amount of tax withholding to deduct from employees' wages.
Direct Deposit
This form allows your earnings to be directly deposited into a checking or savings account. You can find your routing and account numbers on the bottom of a check. To ensure the numbers are accurate, please attach a voided check to this form.
